HPCSA Registration Pathway for International Medical Graduates (IMGs)
Registration with the Health Professions Council of South Africa (HPCSA) under the Medical and Dental Professions Board is a strict legal requirement for foreign-qualified medical practitioners. This pathway guide explains the independent credential verification and registration requirements before any formal application or examination can proceed.
Phase 1: Primary-Source Credential Verification via ECFMG/EPIC
All foreign medical degrees and training documents must be verified from the source before HPCSA application submission. This process begins with ECFMG/EPIC and the World Directory of Medical Schools.
Verification Resources
- Medical School Verification: Confirm the applicant's medical school is actively listed in the World Directory of Medical Schools.
- MyIntealth Account Setup: Establish a secure EPIC portfolio inside the MyIntealth ecosystem.
- Credential Upload: Upload the required documents according to the target practice category.
General Practitioners
Medical School Diploma, Official Academic Transcripts, and Intern Duty Certificates.
Specialists
All GP documents plus Specialist Qualification Certificate and Specialist Registration Certificate/License from the home jurisdiction.
- Verification Dispatch: EPIC must complete verification and send reports directly to the HPCSA. Do not submit the HPCSA application until all reports have been successfully transmitted.
Phase 2: HPCSA Application and Core Document Requirements
Once primary-source verification is confirmed, the next stage is the HPCSA application process and submission of legally compliant documents.

Phase 3: The Board Examination Framework (OSCE & Written)
Strict Attempt Restrictions
Foreign qualified medical practitioners are permitted a maximum of three (3) attempts within a strict two (2) year window to successfully complete the Board Examination.
The Board Examination is administered under the Medical and Dental Board and combines written and practical assessments. Candidates must satisfy each individual component.
Examination Day Rules
- Arrive at the venue exactly one hour before the designated start time.
- Bring two recent passport-sized photographs: one for the written paper, one for the practical OSCE.
- Present the original ID or passport and the official HPCSA administration confirmation letter.
Assessment Structure
Multiple-Choice Questions (MCQs), Extended Matching Questions (EMQs), short-answer responses to complex clinical case presentations, and radiographic diagnoses. Every component requires a passing mark for overall success.
